The proforma is based around the Occupational Performance Model of Australia. Routine Task Inventory-Expanded (RTI-E)

Routine Task Inventory-Expanded (RTI-E) is an assessment of cognitive abilities in the context of routine daily activities. Developed by Claudia Allen in 1989 a manual was later developed by Noomi Katz, PhD, OTR, (Professor of Occupational Therapy at Hebrew University, Jerusalem) in order to encourage others to use this tool in clinical practice, and research. Brookvale Living Skills Assessment

The Brookvale Living Skills Assessment is a great functional assessment used in conjunction with ADL observation. It’s easy to administer with the ‘tick and flick’ form and simple questions. A useful addition is the time use pie graph, where the clinician will draw their daily time use in the first circle as an example and then the client will fill out how they use their day in the second circle.
